PRN Physical Therapist Home Health

Description We are seeking a highly skilled and dedicated Physical Therapist to join our team at HealthONE Healthcare at Home. As a PRN Physical Therapist Home Health, you will play a vital role in providing high-quality patient-centered care to patients in the comfort of their own homes.

The ideal candidate will have a strong passion for creating positive patient interactions and a commitment to delivering exceptional care. You will work closely with our interdisciplinary team to develop and implement individualized care plans that focus on goal achievement, patient engagement, and positive patient experience.

Responsibilities

• Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
• Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
• Provide purposeful and skilled therapeutic care at patient's home, using procedures and techniques to produce a change in condition/function consistent with the physician's plan of care.
• Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
• Prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.

Requirements

• Licensed Physical Therapist.
• Driver's License.
• Bachelor's Degree.
