Responsibilities:
- Conduct comprehensive health assessments and develop individualized care plans for patients in a skilled nursing facility
- Diagnose and manage common acute and chronic medical conditions in post-acute and long-term care patients
- Order and interpret diagnostic tests, including lab work, imaging, and EKGs
- Prescribe medications, monitor treatment plans, and adjust therapy as needed
- Provide patient and family education regarding diagnoses, treatment plans, and preventive care strategies
- Collaborate with interdisciplinary teams, including physicians, therapists, and nursing staff, to ensure coordinated patient care
- Perform minor procedures as needed within the scope of practice
- Document all patient interactions, assessments, and treatment plans in the electronic medical record (EMR) system
- Refer patients to appropriate specialists and coordinate follow-up care as necessary
- Maintain compliance with state and federal regulations, facility policies, and best practices in post-acute care
- Active Nurse Practitioner (NP) or Physician Assistant (PA) License in Arizona
- Completion of an accredited NP or PA program in the U.S. (excludes Puerto Rico)
- 1-2 years of experience in a post-acute care or skilled nursing facility setting preferred
