Location: Mesa / Gilbert / Chandler, AZ

Setting: Skilled Nursing Facility (Post-Acute Care)

Schedule: Part-Time (Saturday & Sunday 0800-1700)

Travel: No

Compensation: $65 - $70 / hr

Status: Contractor / 1099

Job Summary:

We are looking for a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in a skilled nursing facility. In this role, you'll work closely with physicians, nursing staff, and other healthcare professionals to provide high-quality care to patients in a post-acute setting. Responsibilities:

• Conduct comprehensive health assessments and develop individualized care plans for patients in a skilled nursing facility
• Diagnose and manage common acute and chronic medical conditions in post-acute and long-term care patients
• Order and interpret diagnostic tests, including lab work, imaging, and EKGs
• Prescribe medications, monitor treatment plans, and adjust therapy as needed
• Provide patient and family education regarding diagnoses, treatment plans, and preventive care strategies
• Collaborate with interdisciplinary teams, including physicians, therapists, and nursing staff, to ensure coordinated patient care
• Perform minor procedures as needed within the scope of practice
• Document all patient interactions, assessments, and treatment plans in the electronic medical record (EMR) system
• Refer patients to appropriate specialists and coordinate follow-up care as necessary
• Maintain compliance with state and federal regulations, facility policies, and best practices in post-acute care
  
  

Requirements:

• Active Nurse Practitioner (NP) or Physician Assistant (PA) License in Arizona
• Completion of an accredited NP or PA program in the U.S. (excludes Puerto Rico)
• 1-2 years of experience in a post-acute care or skilled nursing facility setting preferred
