- DEA License: DEA license with no restrictions
- Credentialing: Eligible for credentialing with Managed Care Organizations

Requirements
- Licensure: Must be a licensed Nurse Practitioner or Physician Assistant in the State of Arizona
- Education: NP/PA Program Completed in USA (Not Puerto Rico)
