Position Summary:
The Clinical Care Coordinator (CCC) works with patients, families, care providers, and health care team members in partnership with the community to help patients achieve optimal health goals. The CCC will provide patients with support, education, referrals, and resources through comprehensive, coordinated, patient- and family-centered care. The CCC provides culturally appropriate, non-judgmental, compassionate care while maintaining professional accountability and engaging in continuous quality improvement.
Work Schedule:
28 hours per week.
This is an onsite position at our Specialty Center in St. Louis Park.
Required Qualifications:
Minimum of bachelor's degree, or RN, in health-related program required
Preferred Qualifications:
- Experience of two or more years in health-related fields such as mental health, health coaching, childbirth education or community outreach preferred.
- Previous care coordination, intake, clinical quality improvement, discharge planning or case management preferred.
