Nurse Practitioner or Physician Assistant – Primary Care (Adult & Pediatrics)

We are recruiting on behalf of our client, a well-established primary care practice in Monsey , seeking a highly experienced Nurse Practitioner (NP) or Physician Assistant (PA) to provide comprehensive care to both adult and pediatric patients . This role focuses exclusively on primary care — candidates must have a strong background in primary care rather than urgent care.
The provider will work two 12-hour shifts each week (Monday and Wednesday) and must be confident working independently in a busy, community-focused setting.
Key Responsibilities:
Deliver routine and preventive care to patients of all ages
Diagnose and manage chronic and acute medical conditions within a primary care scope
Conduct wellness exams, physicals, and follow-up visits
Develop individualized treatment plans and manage care coordination
Communicate clearly and compassionately with patients and families
Maintain accurate documentation and follow EMR protocols
Qualifications:
Licensed NP or PA in New York State
Extensive experience in primary care for both adults and pediatrics is required
Strong ability to work independently and manage a consistent patient load
Excellent clinical judgment, interpersonal skills, and attention to detail
Passionate about delivering long-term, relationship-based patient care
