Job Overview
The Maternal Infant Health Program is a strength-based home visitation program for pregnant and postpartum mothers in the Pomona area. We are a child abuse prevention program focusing on families who are at high risk for entering the child protective services system due to addiction and/or mental illness. Our goal is to identify and serve pregnant and postpartum women who have known histories of substance use, current substance use, and mothers of substance-exposed infants. We accomplish our goals by surround vulnerable mothers, infants, and families with supportive services that are sustainable and grounded in the community. We provide healing and support by addressing mental health and trauma, offer in-home services, case management, linkage to concrete supports, counseling, education, parenting, baby bonding, support groups and more.
The Clinical Home Visitor will provide strength-based mental health services with high-risk pregnant mothers or parents of an infant 0-24 months old. This is a child abuse prevention program focusing at-risk parents due to addiction or mental illness. Through our collaboration with Pomona Valley Hospital, the Clinical Home Visitor will provide client engagement, home visitation, clinical assessment, individual counseling, psycho-education groups, and prenatal/postpartum workshops.
Key Responsibilities
- Conduct outreach and engagement of pregnant or postpartum mothers at Pomona Valley Hospital, local OBGYN Clinics, or the community.
- Provide home-based (following Covid safety restrictions) therapeutic interventions.
- Complete assessment with families to include – bio-psycho-social, needs assessment, screenings for depression and addiction, family functioning, protective factors, and baby developmental milestones.
- Collaboratively develop a Client Care Coordination Plan based on strengths, observations, and information gathered; Help participants define their goals.
- Help the caregiver gain insight regarding personal history (including trauma history), feelings for the child, and current parenting practices.
- Provide empathetic support and feedback that is trauma informed and culturally competent when working with new moms.
- Work as an active team member in a multidisciplinary team.
- Facilitate psycho-education workshops and parenting groups.
- Avert crisis situations by assisting the family in times of urgent need (risk of harm to child or caregiver, pending child removal, etc.), in partnership with the Case Manager and Supervisor.
- Co-facilitate meetings and engage community leaders in our coalition.
- Responsible for maintaining all clinical documentation at a high standard, completed in a timely manner, and accurate.
- Collect all pre and post surveys and outcome measures.
- Maintain confidentiality in compliance with agency, HIPAA and 42-CFR standards.
Education and Knowledge, Skills and Abilities
- Required: Master’s Degree: MFT or MSW from regionally accredited graduate program in the field of psychology, counseling psychology, or social work.
- Minimum 3 years field-based clinical work experience with at-risk families who have complex needs.
- Bilingual English/Spanish is desirable, but not required.
- Experience with evidence-based, culturally competent, gender responsive, and trauma informed services.
- Willingness to travel to provide field-based services.
- Dependable automobile, California Driver's License, and valid car insurance.
- Must be able to pass background/criminal check.
