Job Overview

    

Project ADAPT is an adult outpatient program that provides culturally competent substance use and mental health services in English and Chinese.  Our program offers a strengths-based, trauma-informed, harm reduction approach.  Services include individual, group and case management services as well as acupuncture and access to free cultural/recreational events.  We also have free community lunches and encourage developing supportive relationships and community.  Our goals are to provide a safe and respectful counseling space for people from all walks of life and to reduce the impact of substance use and mental health conditions that lead to healthy, meaningful lives.

The Substance Use Counselor II/III at Project ADAPT works with a interdisciplinary team to provide substance use treatment, assessing client needs and strengths in support of health and recovery.  The SUD III Counselor provides individual, group counseling using evidence-based curricula in accordance with program specific requirements (Drug Medi-Cal) as well as case management to assist clients in navigating systems of care and connecting to community resources (e.g. vocational, primary care, housing).

         

Key Responsibilities

    

Individual Treatment Responsibilities:

  • Maintains a caseload of SUD counseling clients and facilitates weekly individual counseling sessions with each caseload participant.  
  • Provides clinical support and evidence-based interventions to support clients in meeting their treatment goals.
  • Proactively links clients to both internal and external resources based on their treatment needs and follows up on their progress/status.  

Treatment Setting Responsibilities:

  • Facilitates group counseling sessions, community meetings and teaches classes on various treatment related topics.
  • Performs crisis intervention and communicates with treatment team as unforeseen situations arise.  
  • Plans and facilitates client celebrations and special events.
  • Attends required trainings and meetings related to their position.

Documentation Responsibilities:

  • Collaborates with each caseload client and other available internal and external resources to develop/maintain treatment plans, transition plans, progress notes and appropriate updates in support of the health and recovery needs of the client.
  • Properly documents all aspects of treatment including services and treatment planning in accordance with HIPPA and Drug Medi-Cal billing compliance requirements. This includes completing all consent forms, authorizations to release information, individual and group counseling sessions, discharge paperwork/process, and required assessments in a timely manner.
  • Maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.

         

Education and Knowledge, Skills and Abilities

    

Education and Experience Required:

  • Drug and Alcohol Registration or Certification recognized by DHCS (e.g. CCAPP, CADTP) or Registered with the Board of Behavioral Sciences as AMFT, ASW or APCC.
  • High School diploma or equivalent.
  • First Aid Certified within 30 days of employment.
  • CPR Certified within 30 days of employment.
  • A valid California driver’s license.
  • Culturally competent and able to work with a diverse population.
  • Experience providing individual and group counseling.
  • Experience with clinical documentation (treatment plans, progress notes, etc.).
  • Experience with harm reduction and evidenced-based practices (e.g. CBT, Seeking Safety, DBT, MI).
  • Strong proficiency with Microsoft Office applications, specifically Word, Outlook and internet applications.

 Desired:

  • Bachelor’s Degree in related field.
  • Knowledge of co-occurring disorders and trauma informed treatment.
  • Experience working with criminal justice population.
  • Familiarity/experience with Drug Medi-Cal requirements and billing.

 Background Clearance Required:

  • Must not be on active parole or probation.
