Job Overview
Transportation Care Coordinator is responsible for the transportation needs of clients following all applicable federal and state driving laws. They must ensure all clients are safely transported to and from various location related to their treatment needs, as well as maintain all applicable transportation documentations. Transportation Care Coordinator also supports house functions such as transporting supplies to and from facilities, and cleaning and maintaining weekly inspections of all vehicles.
Transportation Care Coordinator are responsible for safely transporting clients in an agency-provided vehicle to their appointments, participates in Food Bank pick ups and unloading food and supplies, routine vehicle maintenance including weekly inspections, cleaning and pumping gas-maintain trip logs and vehicle inspection forms for the agency vehicles. DMC follow-up documentation: timely documentation of client's medical, mental health and dental appointments into their Welligent chart for billing purposes and uploading documents into the clients chart.

Education and Knowledge, Skills and Abilities
- Valid California Driver's License.
- Ability to clean and maintain agency vehicle insurance coverage.
- Ability to lift up to 30 pounds.
- AOD registered or certified.
- Physical exam plus negative TB screening.
- 1st Aid & CPR certification; live scan clearance.
- Ability to obtain and maintain background clearance with successful discharge from parole, and absence of drug/alcohol related arrest or convictions, for at least 3 years.
- Ability to enter date into various electronic system while maintaining the integrity and accuracy of the data.
