- M-F Schedule
- Case management and Epic experience required
- Patient Ratios : Case Manage 20-25 patient with an expectation of 5-6 visits per day; may use as re-vistor if agency needs change at any time during contract
- Must Have Skills / Experience : Epic / Oasis experience, Home health
- Preferred Skillset / Experience : IV therapy, tube feeds, trachs, compression wraps, port access,ostomies, chest tubes, S / p catheters, lad draw,etc.
- Type of Patients : Adult med surg / oncology / Cardiac / Neurological / Respiratory / Wound Care / GI / GU patient
