The BH Care Coordinator is proactive in the responsibility of providing support to the daily operations of HealthWorks for Northern Virginia Behavioral Health program. The position responsibilities include a wide range of activities related to referrals, coordination, and care management of BH patients at HealthWorks. The position works closely with the BH Director in ensuring timely coordination of care, development of internal and external relationships, and growth of integrated care in the clinic. The BH Care Coordinator will also work closely with BH and Medical providers to ensure that patients are receiving necessary services through integration and appropriate use of resources. The BH Care Coordinator implements policies and procedures developed by the BH Director which ensure an accurate, timely, efficient, and transparent process for BH services. This entails processing referrals, providing follow-up with patient needs, determining the appropriate level of care and referral to outside resources. Data collection around current waitlist, processed and pending referrals is also a responsibility of this position.

Roles & Responsibilities
- Ensures that all referrals sent to BH are processed within 24-48 hours.
- Supports timely response to scheduling patients needing medication management with Psychiatric Nurse Practitioner (PNP).
- Develops and maintains relationships with outside referral sources to ensure that the BH team has up-to-date information about who is accepting new patients, the process for admission, and who qualifies for which service. Ensuring that the team always has adequate up-to-date referral resource information readily available for patient and provider education.
- Coordinates care with special partners such as CSB, MHDDSA, DSS, LAWS and others as needed after securing release of information.
- Clear understanding of warm handoffs in the process of integrated care and completes warm handoffs for referrals.
- Monitor specialty and primary care appointments for the patient and assist the patient in keeping those appointments (reminders, transportation arrangements, etc) as needed.
- Assist patients with filling out applications as needed and coaching them on follow-up.
- Collects and provides monthly reports of referral and schedule data.
- Reviews all referral concerns and brings immediate concerns to BH Director for resolve.
