The Agency Investigator is responsible for responding to incidents involving abuse, serious injury, or death, and taking appropriate steps to safeguard individuals. This includes conducting interviews, collecting evidence, and preserving critical information. The investigator will collaborate with law enforcement, Adult Protective Services, and other relevant entities to ensure thorough investigations are conducted. The role requires responding to incidents as needed, while managing the investigation process with professionalism, urgency, and accuracy.
Essential Duties and Responsibilities:
- Incident Investigation:
Investigate all reported incidents involving abuse, neglect, exploitation, or misappropriation of money or property that result in significant consequences, including:
- Death, serious injury, or physical harm
- Physical or sexual abuse
- Significant pain, intimidation, or mental anguish requiring medical intervention
- Loss of funds or property exceeding $1,000 in value
- Collaboration with Agencies:
Instruct callers to contact Adult Protective Services (APS) and, when applicable, local law enforcement. Explain the role of the Disability & Aging investigator and the investigative process to external agencies.
- Cooperation with Law Enforcement:
Work closely with law enforcement officers to share information and support criminal investigations. Cooperate by providing documents, witness statements, photos, and other relevant items. Attend police questioning of witnesses and take notes or obtain written statements.
- Investigation & Documentation:
Conduct independent investigations as necessary, ensuring the collection of facts and evidence. Maintain consistent and thorough documentation of incidents, investigative findings, and resolutions.
- Case Management & Follow-Up:
Notify management at Healthy Outcomes when an investigation is in progress. Contact law enforcement monthly to inquire about the status of criminal investigations and to ensure proper information sharing.
- Requesting Relevant Documents:
Request and review relevant documents such as incident reports, staffing plans, Individual Support Plans (ISPs), medical records, EMS or hospital records, and training documents to support the investigation.
- Interviews & Site Inspections:
Schedule interviews with the person supported and any witnesses present at the time of the incident. Inspect the scene of the incident, taking photographs and/or drawing diagrams when applicable.
- Reporting & Analysis:
Document and report investigative findings clearly and thoroughly, ensuring all reports are accurate, complete, and submitted within required timeframes.
Minimum Qualifications (Knowledge, Skills, and Abilities):
- Experience:
- 2 to 5 years of experience in law enforcement or as a private investigator.
- 2 to 5 years of experience in criminal investigations or handling major reportable events, or an equivalent combination of work experience and training.
- Investigative & Analytical Skills:
Strong problem-solving and analytical skills, with the ability to independently analyze findings, assess evidence, and recommend solutions.
- Judgment & Decision-Making:
Ability to make sound decisions with urgency, balancing the need for thoroughness with timeliness in high-pressure situations.
- Collaboration & Communication:
Proven ability to collaborate effectively with management, law enforcement, and external agencies. Excellent communication skills, both written and verbal, with the ability to clearly convey technical concepts to diverse audiences.
- Organizational & Time Management Skills:
Strong organizational skills, with the ability to prioritize multiple tasks and manage time effectively. Ability to handle sensitive, time-sensitive, and confidential information with discretion.
- Software Proficiency:
Proficiency with Google Docs, Microsoft Office Suite (Word, Excel, Access, PowerPoint).
- Customer Service & Conflict Resolution:
Excellent customer service and conflict resolution skills, with the ability to interact professionally with clients, witnesses, and stakeholders.
- Independent Work & Initiative:
Ability to work independently, take initiative, and manage investigative tasks effectively.
