Description
This is a full-time on-site role as a Hospice Case Manager located in Burbank, CA. The Case Manager will be responsible for coordinating and managing the care of patients receiving hospice services, conducting assessments, developing care plans, collaborating with healthcare providers, and ensuring quality patient care.
Qualifications
- Case Management, Care Coordination, and Patient Advocacy skills
- Knowledge of Home Health and Hospice regulations and practices
- Strong interpersonal and communication skills
- Ability to work effectively in a multidisciplinary team
- Experience in end-of-life care and palliative care
- Current RN/LVN license and BLS certification
- Experience working in a home health/hospice setting is a plus
Salary : $45 - $50
