We are seeking an experienced social worker with a focus on hospice to join our team! The hospice social worker serves as a resource to patients and/or families according to the plan of care.

Hospice Philosophy:
Hospice care is an interdisciplinary approach designed to meet the needs physical, psychological and spiritual of a person at the end of life with the goal of comfort and not cure. Hospice considers the unit of care to extend beyond the patient and include the family and caregivers as defined by the patient.
Responsibilities:
Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to the hospice program. This included emotional, social, financing and environmental resources and to identify appropriate psychosocial problems, intervention and goals for the patient's plan of care.
Participate in interdisciplinary group meetings and development of the patient's plan of care
Qualifications:
Master's Degree from a school or Social Work accredited by the Council on Social Work Education
1 year experience in hospice, palliative care of home health setting
Job Type: Part-time
Pay: $38.00 - $45.00 per hour
