We are seeking an RN Case Manager to join our Hospice team.

- Develop trusting relationships with patients and their families as an end-of-life care expert.

About the Role:
The RN Case Manager will assume full nursing responsibility for delivering high-quality, patient-centered care. This includes conducting initial evaluations, identifying patient needs, and coordinating care plans.
Requirements:
- One year of clinical experience.
- Current RN licensure in the state of practice.
- Current CPR Certification.
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
