Position Summary: The role of the Licensed Practical Nurse is to work as a member of the primary health care team in a supportive capacity to the Physician and Nurse Practitioners.

Essential Functions:

• Measures and documents patient’s primary problems, including vital signs, basic labs, psycho-social history and medications in order to prepare the patient for examination by the provider, anticipating the need for required supplies and equipment.
• Reports to the provider any significant social or other data obtained during conversation with patient or his/her family.
• Performs general nursing care to patients. Administers prescribed medications and treatments in accordance with nursing standards of practice and established scoped of practice for licensed practical nurses and as prescribed by physician or nurse practitioner.
• Maintains basic instruments and equipment. Performs technical functions as determined by the Medical Director.
• Observes, records, and reports patient’s condition and reaction to drugs and treatments to physicians.
• Attaches lab and radiology results to patient chart for provider review daily.
• Recognizes signs/symptoms and/or patient changes and notifies Provider.
• Inventories, orders and restocks supplies as needed.
• Documents care plans and self-management support flow sheets
• Follows up on referrals, including resending paperwork if referral has not been received, ensuring that an appointment has been made. Documents all updates in Referral Management.
• Calls, reschedules and prepares letters to “no show” patients. Return patient calls/voice mails in a timely manner. Documents this process.
• Provides patient education to clients.
• Reviews medical records for completeness of data prior to the visit by scanning the last note written by the provider and ensuring that lab data or other information is contained within or has been returned for the present visit.
• Tracks, records and scans radiological procedure orders for physician. Seeks authorizations of procedures from insurance companies on behalf of patients.
• Actively participates in identification and determination of conditions to ensure high quality care to clients/patients and their families.
• Attends required meetings and participates in health center work as requested.
• Administers immunizations per CDC guidelines.
• Conducts care team huddles daily for efficient communication among the team.
• Participates in Population Health Management activities.
• Participates in tracking hospitalized patients and ensuring timely follow-up.
• Performs related duties as required.

Qualifications

Education: Graduation from an accredited LPN program.

License: Current Florida LPN license and certification to administer medication.

Special skills/knowledge required: Ability to use the nursing process, as evidenced by physical and psychosocial observation skills, interviewing techniques, communication and interpersonal skills, problem solving and clinical judgment. Maintain patient confidentiality. Spanish fluency is desirable, but not essential.
