We are hiring Staff to provide services under Coordinated Career Pathways (CCP)
Qualifications: -
1) A bachelor's degree and a minimum of 3 years of experience in the management of a human services delivery system and ACRE certificate with emphasis on CE or Association of People Supporting Employment First 9APSE) Certified Employment Support Professional (CESP) credentialed and Certificate in Person Centered Training.
OR.
2) An Associate level degree with Five years of experience in a human services delivery system and ACRE certificate with emphasis on CE or Association of People Supporting Employment First 9APSE) Certified Employment Support Professional (CESP) credentialed and Certificate in Person Centered Training (PCT).
If you have qualifications 1 or 2 above without the ACRE or CESP and (PCT), and are willing to enroll for and complete the ACRE and PCT OR (CESP) credentialed and PCT within 1month of hiring we would hire you as a CFS worker with reimbursement rate of $22.50 per hour and move you to CCP with the rate of $24 per hour upon completion and presentation of ACRE/CESP and Person Centered Planning certificates.

Job Description: -
Ø Conducting the initial Assessment
Ø Conducting Discovery
Ø Assist Individual and circles of supports understand career options.
Ø Assist Individual and circles of supports make best career choice.
Ø Development of Person Centered Career Plan (PCCP)
Ø Meet with IPP team to discuss IPP, integrate PCCP into IPP
Ø Assist Individual transition into career choice.
Ø Research and connect Individual with all needed generic services and resources.
Ø Outline all required non-generic services Individual may require, present to the Regional Center for consideration and approval.
Ø Assist Individual transition into receiving all approved services.
Ø Coordinate all services Individual is receiving for consistency.
Ø Monitor the Individual’s performance for compliance with IPP.
Ø Conduct monthly review of performance and compliance with IPP.
Ø Meet IPP team if review of IPP is necessary.
Ø Provide quarterly reporting to DDS
Ø Assisting Individual develop Employment Profile
Ø Job Search Planning
Ø Job Development and Negotiation
Ø Job Customization
Ø Research and coordinate generic services and resources the Individual may require..
Ø Coordinate all ongoing supports the Individual may be receiving.
Ø Assist Individual transition into receiving all approved services.
Ø Monitoring Performance of Individual per IPP
Ø Monthly Review of Customized Employment
Ø Coordinating other Supports (generic and non-generic) Necessary to Sustain Employment
Ø Attending IPP Meeting if Necessary
Ø Quarterly Reporting
