Job Title: Nurse Practitioner, Per-Diem

High-level Job Description
The Nurse Practitioner provides hospice care through the evaluation and assessment of the health history, health promotion, diagnosing medical conditions, management of health problems by directing and developing the plan of care, prescribing medications or treatments, coordinating and collaborating with patients, families and other healthcare practitioners. Under the supervision of a physician, the Nurse Practitioner visits patients, completes physical assessments, performs face to face assessments and evaluates hospice eligibility as assigned in consultation with the hospice interdisciplinary team.
Pay Rate and Benefits
Highly compatible pay rate based on relevant work experience and qualification. We offer employee benefits including Paid Sick Leave and 401K retirement plan with optional company matching.
Visit Rate: $200/visit
Hourly Rate: $80-$120/ hour
Qualifications/Requirements
1. Graduation from an accredited School of Nursing.
2. Master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience.
3. Has a current DEA license.
4. Completion of an accredited Nurse Practitioner Program.
5. Current nursing licensure in State of California and BLS certification.
6. Accreditation from an approved certifying body for advanced practice nursing as required by State.
7. Certification in Hospice and Palliative Nursing (CHPN) preferred.
8. Excellent observation, verbal and written communication skill, problem solving skills, mathematical skills; nursing skills per competency checklist.
9. Prolonged or considerable walking or standing. Able to lift, position and/ or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
10. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
11. Able to cope with emotional stress tolerant of different lifestyles, cultures, beliefs, and values.
12. Knowledge of and commitment to hospice philosophy of care.
13. Experience in hospice and home care are advantages.
14. Bilingual Chinese/English Speaking welcomed but not required.
15. Compliant with Covid-19 healthcare personnel requirement in all City, County, State and Federal level.
