Job Overview
This Hospice Registered Nurse (RN) position is a rewarding opportunity to make a difference in the lives of those facing life-limiting illnesses. As a member of our interdisciplinary team, you will provide compassionate care, education, and support to patients and their families.
Responsibilities
- Initial Nursing Evaluation: Conduct thorough assessments within 48 hours of referral to determine eligibility for hospice services.
- Patient-Centered Care: Identify patients' physical, psychosocial, and environmental needs, reassessing as needed every 15 days.
- Care Planning: Develop and implement individualized plans of care in collaboration with the patient, family, attending physician, and other healthcare professionals.
Maintain accurate clinical notes, document problems, goals, interventions, and patient/family responses to care. Provide education and guidance to patients and families on self-care techniques, promoting independence and comfort.
Requirements
- Clinical Experience: One year of RN experience required.
- Licensure and Certification: Current RN licensure in the state of practice and current CPR certification.
- Transportation: Reliable vehicle or access to public transportation required.
Additional State-Specific Requirements
- In Louisiana, two years of full-time experience working as an RN are required; however, two years of full-time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse.
