We are seeking a Hospice Registered Nurse for a PRN position at Heart of Hospice, part of LHC Group.

Key Responsibilities
Assume full nursing responsibility for delivering patient care, including continuous evaluation and improvement of personal and professional performance.
- Conduct initial evaluations within 48 hours of referral to determine hospice eligibility
- Identify and reassess patients' physical, psychosocial, and environmental needs every 15 days
- Develop and coordinate patient care plans
- Document problems, goals, interventions, and patient response
- Collaborate with patients, families, physicians, and other healthcare professionals
- Provide education and guidance on self-care techniques when necessary
- Maintain accurate clinical notes
Requirements
To be successful in this role, you will need:
- One year of clinical experience
Licensure and Certification
You must possess:
- Current RN licensure in your state of practice and one year of clinical experience
- Current CPR certification
- A valid driver's license, vehicle insurance, and access to reliable transportation or public transit
Additional Requirements
Please note the following state-specific requirements:
- LA: A minimum of two years of full-time experience working as an RN; however, two years of hospice care experience as an LPN may be substituted
