Take Your Nursing Career to a New Level
In this role, you will assume full nursing responsibility for delivering high-quality patient care. You will continuously evaluate your performance and make necessary adjustments to increase productivity and quality of care.
- Conduct initial nursing evaluations to determine patient eligibility for hospice services within 48 hours of referral.
- Identify patients' physical, psychosocial, and environmental needs, reassessing as needed every 15 days.
- Develop and coordinate personalized plans of care.
- Document patient progress, goals, interventions, and responses to hospice care.
- Collaborate with patients, families, physicians, and other healthcare professionals to deliver comprehensive care.
- Provide education and support to patients and families on self-care techniques.
- Maintain accurate and relevant clinical notes on patients' conditions.
Experience Requirements
- 1 year of clinical experience.
License Requirements
- Current RN licensure in the state of practice and 1 year of clinical experience.
- Current CPR Certification.
- Valid driver's license, vehicle insurance, and reliable transportation or access to public transportation.
Additional State-Specific Requirements
- LA: minimum 2 years of full-time experience as an RN, or 2 years of hospice care experience as a licensed practical nurse.
