Job Title: RN Case Manager in Hospice

We are seeking a compassionate and skilled RN Case Manager to provide end-of-life care with empathy and understanding.

This opportunity allows you to develop trusting relationships with patients and their families, valuing and respecting them throughout the care process.

Key Responsibilities:

• Assume full nursing responsibility for delivering patient-centered care.
• Conduct initial nursing evaluations within forty-eight hours of referral to determine eligibility for hospice services.
• Identify patients'/families' physical, psychosocial, and environmental needs, reassessing as needed.

Experience Requirements:

• One year of clinical experience is required.

License Requirements:

• Current RN licensure in the state of practice and one year of clinical experience.
• Current CPR Certification.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
