Responsibilities include providing case management and psychosocial rehabilitative training and other activities mandated by HHSC contract or other rules for individuals with MI as deemed necessary in accordance with the recovery plan. Assessing consumer needs utilizing the Uniform Assessment and other designated assessment tools for the development of comprehensive recovery plans. Completes the community living options process and makes appropriate referrals. Follows up with refusals in order to educate and offer services as required. Provides immediate response to emergency interventions when necessary and advocates for consumers. Accompanies consumers to annual, quarterly, and called IDT planning meetings. Responds to alerts and maintains tracking documentation. The person in this position will be responsible for the completion of PASRR Level I and PASRR Level II Evaluations for people admitting to or residing in Nursing Facilities who are identified as potentially having Mental Illness. These evaluations require face to face interviews with the individuals, their family members, Nursing Facility staff and others who know the individual, as well as research of existing medical records, for comprehensive discovery of pertinent information to accurately complete the evaluations.

EXAMPLES OF WORK PERFORMED::
- For individuals with MI, completion of the ANSA to determine a service package appropriate for the individual’s needs; collaborative development of an individualized recovery plan with each consumer.
- Provision of case management and or psychosocial rehabilitative training as deemed necessary and in accordance with the individual’s treatment plan.
- Coordination of IDT meetings following the PASRR evaluation and Initial IDT meeting in order to discuss the specialized services recommended for the individual. Attendance and/or coordination of annual, quarterly, and called IDT meetings for those individuals who are receiving ongoing services;
- Attendance at the annual IDT meetings for individuals who have previously refused PASRR services.
- Complete PASRR Level I and Level II Evaluations;
- Provide face to face interviews of individuals, family members, NF staff and others in order to gather information pertinent to completion of the evaluations;
- Data entry of the completed evaluations into the TMHP portal;
Experience and Education::
- Position requires a Bachelor’s or Master's degree from an accredited college or university in an appropriate related field.
- Individual will be certified as a QMHP (Qualified Mental Health Professional).
- QMHP credentialing and annual renewal is required through training and competency evaluations for employment in this position.

Knowledge, Skills, and Abilities::
- The individual in this position will have a working knowledge of both trauma informed care and person-centered planning, knowledge of Mental Health diagnoses, and knowledge of the community resources available to people with MH and/or IDD.
- The individual in this position must have a strong work ethic, be well-organized, and must be able to adapt to rapid changes in work assignments.
- The individual in this position must also be able to work in harmony with a small team, be able to connect with and communicate clearly with others, and have strong interviewing skills.
- Must have and maintain a level of general health that permits the performance of job duties.
- Must be able to successfully participate, perform, and complete the skills required for CPR, First Aid, and PMAB certification.
- Ability to travel locally 70% and regionally 10%.
- Ability to lift up to 30lbs.
