A Wraparound Case Manager provides intensive evaluation, consultation and intervention with youth and families, within Heart of Texas Behavioral Health Network catchment area, who are participating in wraparound services and/or YES waiver. Also participates in conducting psychosocial programming with this population as part of a clinical team.

EXAMPLES OF WORK PERFORMED::
- Works with youth and their families in mental health, home or office settings
- Provides case management and some skills training with children and families
- Performs assessment and psychosocial evaluations and makes treatment recommendations
- Refers families of clients to appropriate social welfare and health agencies to obtain assistance
- Conducts family interview with every family to create a family story
- Develops a crisis/safety plan with the family and updates as needed
- Helps identify other community and individual supports that can be used by the family to achieve their care plan
- Creates a Child and Family team for each youth and family
- Is the primary contact person for each family in the wraparound system on their caseload and will be required to develop a plan of care for that individual family
- Regular attendance at Family team meetings
- Must complete Wraparound training
- Must comply with all required documentation and data collection to support the program’s implementation and evaluation strategies
- Other work as required
- Experience as set forth under Education.
- Must has current valid driver’s license and current and continued proof of liability insurance.

Education::
Bachelor’s or Master's Degree in Psychology, Social Work or related field.
KNOWLEDGE, SKILLS AND ABILITIES: :
- Knowledge of emotional problems and treatment approaches Ability to establish and maintain positive working relationships with clients and colleagues.
- Good work habits, organizational skills, and time management skills.
- Familiarity with computer operation for clinical documentation.
- Effective verbal and written communication skills.
- Ability to design and solve problems
- Ability to develop and deliver case presentations.
- Must have level of general health permitting stooping, bending, climbing stair, ability to lift up to 15 lbs, and participating in moderately active recreational activities.
- Physical ability to perform CPR and PMAB skills.
- Ability to create effective relationships with individuals of different cultural beliefs
- Ability to transport clients. Local travel 80% and regional travel 20%
