Performs mental health and support services for individuals who are currently homeless. Work involves conducting interviews, providing intensive case management support to homeless individuals; coordination with area social service agencies having contact with the homeless; involvement in local homeless organizations. Works under moderate supervision of the Program Supervisor with limited latitude for the use of initiative and independent judgment in carrying out details of the work.

EXAMPLES OF WORK PERFORMED::
- Builds relationships with local shelters and other community agencies interacting with the homeless population
- Provides on-site relationship building with homeless individuals
- Coordinates referrals to community-based providers
- Active in local homeless organizations
- Prepares progress notes and other required documentation
- Requires successful experience in the social service field and a working knowledge of mental illness.

Education:
Graduated from an accredited college or university with a Bachelors Degree and a major in behavioral, social sciences, humanities, education, or related field.
Knowledge, Skills, and Abilities:
- Knowledge of mental illness and emotional components of physical illness
- Ability to form positive relationships with both homeless individuals and other social service agencies
- Demonstrate skill in interviewing and reporting results of interviews
- Must have excellent written and verbal communication skills
- Must be able to travel locally and transport clients
- Must be able to lift up to 30lbs
- Must have excellent time management and excellent organizational/filing skills
- Ability to prepare reports and make recommendations for administrative use
- Must have excellent documentation skills.
