A Case Manager provides evaluation, consultation and intervention with youth and families within the community. Also participates in conducting psychosocial programming with this population as part of a clinical team.

- Works with youth and their families in school, mental health, community, home or office settings.
- Works with community and family for the purposes of creating and maintaining individualized service arrays for families.
- Provides case management and some skills training with children and families.
- Makes assessment and psychosocial evaluations and recommendations.
- Refers families of clients to appropriate social welfare and health agencies to obtain assistance.
- Performs Related Work as Required.
- Must have current valid driver’s license and current and continued proof of liability insurance.
- Must have a Bachelor's or Master's Degree in Social work or related experience.
- One year of case management experience Preferred:.
