What are the responsibilities and job description for the Hospice CEC position at Heart of Texas Hospice?
Heart of Texas Hospice is looking to add a Hospice Marketing person (ie Community Education Coordinator - CEC) to join our growing team in the Pearland/Southeast Houston areas! The CEC is primarily responsible for increasing our hospice census. This is achieved by building and maintaining relationships with healthcare providers, referral sources, and families, as well as informing the community of our hospice services.
Responsibilities and Duties
- Develops various channels of referrals through the selection, acquisition, retention, and growth of business partnerships with referral sources.
- Consistently visits, communicates, and monitors provider, community, customer, payer and patient perceptions of Heart of Texas Hospice as a high-quality provider of services.
- Conduct educational in-services and community outreach to promote our superior hospice care services.
- Educates patients and their families of hospice expectations.
- Work closely with the clinical operations team to ensure seamless care transition coordination from an acute care setting to home.
Qualifications:
- Clinical, hospice, and/or home health marketing experience preferred but not required.
- Driver’s license
