What are the responsibilities and job description for the Hospice Liaison position at Heart of Texas Hospice?
Heart of Texas Hospice is looking to add a Hospice Liaison for our growing hospice in the Georgetown area! Working on a full-time basis, the Hospice Liaison is primarily responsible for increasing our hospice census. This is achieved by building and maintaining relationships with healthcare providers, referral sources, and families, as well as informing the community of our hospice services.
Responsibilities:
- Strategically develops various channels sources of referrals through the selection, acquisition, retention, and growth of business relationships in the community.
- Consistently visits, communicates, and monitors provider, community, customer, payer and patient perceptions of Heart of Texas Hospice as the highest-quality provider of Hospice services in the area.
- Conduct educational in-services and community outreach to promote the benefits of hospice care, as well as what makes Heart of Texas Hospice unique as a provider of these services.
- Educates patients, and their families, on the benefits available to them for hospice and what to expect as a hospice patient.
- Work closely with the clinical operations team to ensure seamless care transition and coordination from multiple care settings.
Qualifications:
- 2 years of Hospice marketing experience
