What are the responsibilities and job description for the Hospice LVN position at Heart of Texas Hospice?
Heart of Texas Hospice is looking to hire a Full Time LVN to join our ever growing team in the Killeen/Temple/Lampasas areas.
Responsibilities:
- Conducting on call visits, routine visits, and/or follow-up visits at home or other facilities settings.
- Coordinating, assessing, and implementing patients' care.
- Providing accurate and timely documentation of patient services to reflect plans of care.
- Monday thru Friday work week.
- Occasional On Call 3-4 times per month Monday - Thursday.
Qualifications:
- LVN licensure (current, unrestricted) in the State of Texas
- Hospice experience preferred but not necessary
