EXAMPLES OF WORK PERFORMED :
- Provides short-term counseling and skills training services to patients and their families in rural integrated health settings
- Provides care coordination, system navigation, crisis intervention, and community resource linkage and referrals for individuals served
- Work with interdisciplinary team to coordinate both behavioral and physical health care in rural primary care settings
- Complete all documentation including treatment plans, treatment plan reviews, progress notes, discharge summaries and other relevant documents

Education and Experience
Hours : M-F 8AM-5PM with other as needed
Salary : Master's $48,150-$52,965 Annual DOE with an additional Regional Stipend $5000
Licensure $64,000 Annual with an additional Regional Stipend $5000
