POSITION SUMMARY
Help Me Grow Kent (HMGK) is an outreach and navigation service that connects families with children ages 0-5 to existing resources within the community. HMGK also helps caregivers understand the development of their child through the Ages and Stages Questionnaire. These essential functions happen through telephone intake, incoming referrals from Partner Organizations, and through the HMGK Website.
The Help Me Grow Kent (HMGK) Care Coordinator is responsible for acting as a point of contact for incoming clients, document referrals, provide timely follow-up for clients who have been referred for services, generate reports, and various other reasonable essential functions for HMGK.
KEY RESPONSIBILITIES / ESSENTIAL DUTIES OF POSITION
- Respond to calls and online entries from parents, childcare providers, educators, health professionals and social service agencies about child development issues and services.
- Provide crisis intervention when appropriate, assess and connect clients with services through referrals and conference calls, advocate as needed and provide care coordination and follow up.
- Utilize a "no wrong doors" approach when assisting clients.
- Make referrals to Partner Organizations following appropriate operations and protocols.
- Conduct follow-up care coordination calls.
- Support the processing of completed developmental screenings and follow up with caregivers as appropriate.
- Maintain the HMGK client database, the HMGK Partner Organization database, and Brooke's Platform ASQ databases.
- Help identify, document, and resolve service gaps and barriers.
- Use 2-1-1 and Help Me Grow Kent systems to access information about resources, document calls, collect required data and produce reports.
- Attend and participate in meetings, including internal, care coordination, regional and partner meetings.
- Attend outreach and screening events.
- Travel to a national Help Me Grow conference may be required once per year.

Minimum Salary for the role :
All roles within HWMUW have pay ranges that commensurate with the knowledge, skills, and abilities of the successful candidate. The starting hourly pay range for this position is $22.00 - $26.00.
