Position Summary
The Admissions and Program Development Specialist is responsible for managing all aspects of outreach, client screening, and intake coordination for new admissions into Heart to Heart Home Care’s intellectual and developmental disability (IDD) services in Pennsylvania. This role ensures a seamless admissions process, from initial contact through documentation completion, while also supporting new program development initiatives.
Key Responsibilities
- Lead outreach efforts to referral sources, case managers, and families to generate new admissions.
- Screen prospective clients for suitability in available programs by conducting Meet & Greets and reviewing support needs.
- Serve as the primary point of contact for all new admissions, guiding families and referral sources through the process.
- Process all required documentation for new client admissions and new program development, ensuring compliance with Medicaid waiver regulations and state guidelines.
- Track and analyze admissions trends, identifying opportunities for program expansion and service enhancements.
- Collaborate with operations and leadership to ensure smooth transitions for new clients and to support program growth initiatives.
Qualifications & Skills
- Experience in admissions, intake coordination, or program development within the IDD or healthcare/social services field.
- Familiarity with Pennsylvania’s ODP system and Medicaid waiver programs.
- Strong outreach, networking, and relationship-building skills with referral sources and families.
- Excellent organizational and communication skills, with attention to detail in documentation.
- Proficiency in Microsoft Office and tools for tracking leads and admissions.
- Ability to travel within Pennsylvania as needed.
