Position Summary
Transform lives as a Direct Support Professional in our group home setting. You'll provide essential support to individuals with disabilities while promoting their independence, dignity, and personal growth. Key Responsibilities
- Provide personalized assistance with daily living activities including hygiene, meals, and medication management
- Monitor health status and communicate effectively with healthcare professionals
- Collaborate on developing and implementing individualized care plans
- Facilitate social and recreational activities that encourage community integration
- Maintain a safe, clean, and welcoming home environment
- Support clients with mobility assistance and physical needs
- Document client progress and significant observations according to established protocols
- Build trusting relationships while respecting clients' privacy and personal choices
Qualifications
- High school diploma or equivalent
- Valid NJ non-probationary driver's license
- Social Security card, birth certificate, or passport
- CPR/First Aid certification (training available through Heart to Heart)
- Experience working with individuals with disabilities preferred
