Responsibilities:
- Records & reports observations.
- Conducts telephone follow-ups.
- Performs other related functions as requested by the supervising nurse within the scope of practice and related experience.
- Participates in performance improvement activities.
- Performs other job related duties as may be required or directed by the nursing supervisor.
Requirements:
- Must be fluent in conversational Spanish.
- Participate in performance improvement activities.
- Performs other job-related duties as may be required or directed by the nursing supervisor.
- Must be COVID vaccinated.
- Home care experience a PLUS.
