Job Overview
As an In-Home Direct Support Professional, you will play a vital role in enhancing the quality of life for an individual in the I/DD community. Your primary responsibility will be to provide compassionate care and support to a client in their home setting, ensuring their daily needs are met while promoting independence and dignity. This position requires a patient and dedicated individual who is committed to fostering a positive environment for those they serve.
Responsibilities
- Assist client with daily living activities, including personal hygiene, meal preparation, and medication administration.
- Monitor vital signs and report any changes in health status to the appropriate healthcare professionals.
- Develop and implement individualized care plans in collaboration with the client, their family, and other team members.
- Provide support in social and recreational activities to encourage community involvement and personal growth.
- Maintain a clean and safe living environment for the client by performing light housekeeping duties as needed.
- Document client progress and any significant changes in behavior or health in accordance with established protocols.
Skills
- Experience working with individuals with disabilities is highly desirable.
- Strong understanding of patient monitoring techniques and vital signs assessment.
- Ability to prepare nutritious meals while considering dietary restrictions or preferences of the client.
- Knowledge of caregiving principles.
- Excellent communication skills to effectively interact with clients, families, and team members.
- Patience, empathy, and a strong commitment to providing high-quality care.
- Ability to work collaboratively within a team environment while also being self-motivated.

Requirements
- Valid NJ non-probationary driver's license
- Proof of valid auto insurance
- SS card, birth certificate, or passport
- High school diploma or equivalent
- CPR/FA certification if completed (if not completed, Heart to Heart offers CPR classes)
