Bereavement Coordinator - Part Time

Hourly Rate: $30 - $35 per hour
What You Must Have:
- High School Graduate, Degree/License in counseling preferred
- Experience in bereavement and grief counseling; preferably in hospice and palliative care setting.
- Current State Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
- Experience with EMR software is a plus

What You Will Do:
- Assess hospice family to identify bereavement needs as evidenced by documentation and community resource referrals
- Promote Agency philosophy and administrative policies to ensure quality of care
- Collaborate with interdisciplinary group to promote coordination of bereavement care plan and interventions
- Provide continued bereavement support to hospice families as evidenced by documentation
- Participate in QAPI
- Maintain current Bereavement Coordinator documentation, files and plan of care follow-up
- Training and monitoring volunteers that support the bereavement program
- Participate in all mandatory staff in-services and meetings
- Timely submission of all documentation
- Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
- Performance of other duties as required
