This mission is accomplished through a unique blend of prevention, placement, and housing programs all designed to help elders find and succeed in homes of their own. To this end, all housing operated by Hearth provides a creative array of supportive services that assist residents to age with dignity, regardless of their special medical, mental health, or social needs. Hearth believes these goals are best accomplished through respect for elders and staff, with the desire to see both achieve their highest degree of potential.

Overview:

The Adult Clinical Community Services (ACCS) Outreach Worker at Hearth plays a key role in supporting the integration and stability of Persons Served (PS) in the community. This position is responsible for delivering non-clinical Treatment Plan interventions, supporting skill development, crisis prevention, and connecting PS to community resources. The ACCS Outreach Worker serves as an integral part of the Hearth interdisciplinary team, working alongside clinicians, social workers, nursing staff, and housing specialists to promote long-term success for PS.

  • Location: Hearth’s Supportive Housing facilities (e.g., Olmstead Green) and community-based settings.
  • Caseload: Assigned based on program needs and client acuity.
  • Reports To: ACCS Team Leader.

Essential Functions:

Direct Client Support & Skill Development

  • Provide hands-on support to PS in developing skills for daily living, medication adherence, health management, and socialization.
  • Assist PS in accessing community resources, such as public benefits, employment services, and healthcare providers.
  • Support PS in maintaining housing stability, particularly within residential and supportive housing settings, by working in collaboration with property management and care teams.
  • Accompany PS to medical, psychiatric, and other essential appointments as needed.
  • Use evidence-based approaches such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care to promote engagement and independence.

Crisis Intervention & Risk Prevention

  • Identify early warning signs of crisis and implement preventive strategies in coordination with the clinical team.
  • Assist in crisis de-escalation by providing immediate, on-the-ground support and coordinating with emergency services when necessary.
  • Participate in Safety Plan and Crisis Prevention Plan development, ensuring PS have strategies to manage distress.

Collaboration & Community Engagement

  • Work closely with clinical team members, including social workers, case managers, and nurses, to provide wraparound support for PS.
  • Establish and maintain relationships with community service providers, ensuring PS have access to mental health, substance use, financial, and legal resources.
  • Act as a liaison between Hearth and external agencies, advocating for PS needs.
  • Engage directly with residential and supportive housing programs to ensure continuity of care for PS in various living situations.

Documentation & Compliance

  • Maintain accurate and timely documentation of PS interactions and progress in Hearth’s internal tracking system.
  • Support team compliance with DMH and Hearth program requirements by documenting interventions, progress notes, and referrals.
  • Participate in interdisciplinary team meetings to discuss PS progress, challenges, and care plan updates.

Other Duties

  • Provide orientation and support to new staff and PS to ensure smooth transitions into the ACCS program.
  • Attend required trainings and staff development programs.
  • Share on-call responsibilities for after-hours support as assigned.

Qualifications & Requirements:

Education & Experience

  • High school diploma or equivalent required; Bachelor’s degree in Human Services or related field preferred.
  • Experience working in residential and/or supportive housing settings strongly preferred.
  • 1-2 years of experience working with individuals with psychiatric disabilities, substance use disorders, or housing instability preferred.
  • Experience working with older adults or homeless populations strongly preferred.

Skills & Competencies

  • Commitment to Hearth’s mission of supporting vulnerable elders.
  • Strong problem-solving, crisis intervention, and communication skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Knowledge of engagement strategies to support PS in maintaining housing, treatment, and social connections.
  • Sensitivity to the cultural, religious, ethnic, disability, and gender issues impacting PS.

Certifications & Additional Requirements

  • CPR and First Aid certification (must obtain within two weeks of hire).
  • Medication Administration Program (MAP) certification (must obtain within 150 days of hire).
  • Valid driver’s license and reliable transportation required for community-based work.
  • Must be able to pass background checks, reference checks, and employment verification.

Physical Demands & Work Environment

  • Ability to stand, walk, bend, kneel, and climb stairs throughout the workday.
  • Ability to lift up to 25 pounds using proper lifting techniques.
  • Comfortable working in varied environments, including client residences, office settings, and outdoor locations.
