Hearthstone Communities is in growth mode and currently seeking a Registered Nurse (RN)/ Licensed Practical Nurse (LPN) to join our team! The Registered Nurse/Licensed Practical Nurse (LPN) is responsible for supervising the nursing care of residents on assigned units. They will provide appropriate evaluation of resident condition/and documents, communicating vital information to appropriate personnel. The LPN ensures the delivery of nursing care on the unit is consistent with facility standards, and established policies and procedures. Responsibilities:
- Supports and promotes the nursing department functions on the unit by following established policies and procedures.
- Is knowledgeable and responds to nursing and building emergencies according to established policy and procedure. Is knowledgeable and responds appropriately to nursing/facility emergencies according to established policies and procedures.
- May serve as Assigned Supervisor for various nursing department personnel. As such, conducts employee performance reviews, disciplinary conferences, and exit interviews in a timely manner
- Assigns duties and responsibilities to unit staff members in a manner that facilities optimum delivery of care to residents.
- Conducts regular rounds on the unit and notifies appropriate personnel of findings.

Qualifications:
- Licensed currently as a Registered Nurse/Licensed Practical Nurse in good standing. Maintenance of licensure requirements required.
- Long term care experience preferred.
