Job purpose
The Family Medicine Advanced Practice Registered Nurse / Nurse Practitioner (APRN) is responsible for providing comprehensive primary care and management of patients with acute and chronic conditions in a collaborative care team setting. The APRN will develop their own patient panels and can grow their practice to meet the needs of the community in line with the APRN’s interests and passions. The APRN plays a key role in expanding the development of an integrated health care model for Heartland Community Health Center.
Duties and responsibilities
- Participate in the care of clinic patients in an independent and interdependent role, diagnosing and treating patients with acute and chronic issues. Schedule consists of an average of 36 patient facing hours per week.
- Consult with physician when needed to construct a comprehensive plan of care and referring patients to specialty care when necessary;
- Regularly incorporate patient education into patient visits and monitor and direct the progress of patients with chronic disease.
- Collaborate with all members of the health care team including behavioral health, dental services, and social work to provide holistic care to patients;
- Responsible for accurate and timely documentation in the patient’s electronic medical record and locking record in a timely manner as specified in policies and procedures;
- Utilize evidence-based guidelines in practice;
- Guide and assist the compliance officer to maintain standards and assist annually with the review and revision of policies and procedures;
- Work with community partners to ensure proper follow-up of clinic patients after hospitalization or mental health evaluation;
- Attend mandatory staff meetings or other meetings as needed;
- Assist in planning to evaluate performance levels, quality of service, productivity and future needs;
- Operate computer and electronic health records competently;
- Must be able to receive, interpret and follow verbal and written communications;
- Good communication skills required;
- The Nurse Practitioner will occasionally be assigned projects and duties by the CEO, CCO or COO in areas such as patient outreach, patient education or quality assurance/quality improvement.
- Occasionally participate in public relations on behalf of Heartland.
- Limited telephone only on-call services as needed (typically 3- 5 nights/month)
Qualifications
- Current Kansas RN license
- Current Board Certification
- Current Kansas license to practice as an APRN
- Current Basic Life Support certification
- N95 FIT test required
- Current Tuberculin skin test within one year or QuantiFERON Gold within two years
- Minimum one year of APRN experience preferred, with a strong background in acute or primary care
- Renew licenses and certifications upon expiration, as required
- Computer skills and ability to navigate Electronic Health Record software

Details
- Hours Monday – Friday ranging from 7:30am – 7pm. Shifts are generally 8-10 hours in length of direct clinic care and duties.
