SUMMARY DESCRIPTION OF WORK

The Care Team Specialist works with clients to provide coordination of services of which they are enrolled. They are the point person to ensure client’s team, both internally and with outside providers, is communicating on a regular basis to ensure optimal care. They help refer to other resources or programs when needed. They are responsible for enrollment into programming and on-going documentation required for the Certified Community Behavioral Health Clinic (CCBHC).

Compensation: wage is based upon total years of relevant experience.

Work Schedule: 37.5 hours per week, Monday through Friday, 8:30 a.m. to 5:00 p.m.

MINIMUM QUALIFICATIONS

  • Bachelor’s degree in psychology, social services, sociology or other related fields desired or comparable work experience preferred
  • One year experience in health and human services, or related field

Essential Duties and Responsibilities

  • Document client contact, Release of Information, consents for treatment, and any additional documentation that may be needed.
  • Check for completion of other assessments agency wide (PHQ-9, DLA-20, Columbia Suicidality Rating Scale if applicable)
  • Works directly with member to assess holistic health care and social service needs, client strengths, abilities and wants.
  • Develops care coordination plan with the client that is realistic, achievable, goal oriented, and focused on client needs and wants.
  • Bring together client’s team to include family members, treatment and healthcare workers, legal team, etc.
  • Interacts with primary care providers, behavioral health providers and other social service providers to make needed connections for services, provide a conduit for information sharing and organizes case conferences to bring providers together to review the care of mutual members.
  • Works within the team model, utilizing the expertise of the team members when appropriate for each client.
  • Coordination assessment upon admission and required timeline for state reporting.
  • Be an advocate for the client and help ensure engagement stays consistent.
  • Refer to appropriate resources/services, which meet the client’s needs to include physical health, behavioral health/substance use, social services, housing, education, and employment.
  • Hold monthly contact either face to face or via telehealth.
  • Create crisis intervention plan with assistance from the client to ensure natural supports are identified, as well as self-help coping skills.
    • Create, maintain and share as appropriate a dynamic self-care plan.
    • Strive to make connections between the agency and the larger community whenever possible in order to contribute to the agency’s ongoing fundraising and friend-raising efforts.
    • Essential functions of this job are to be performed on company physical work site or designated workspace.
