Job Summary
This is a professional, management position who will oversee the coordination and implementation of services for mental health, substance abuse and medical care for dependent children and families in Circuit 10 under the Sunshine State Health Plan and other Managed Medical Assistance MMA plans.
ESSENTIAL FUNCTIONS:
· Establishes and maintains positive working relationship with HFC internal and external customers
· Manage the Integrated Health department that consists of 6 positions: Behavioral Health Specialist, Inter-Agency Specialist APD; Inter Agency Specialist DJJ, Nurse Care Specialists (2) and Case Management Support Staff
· Follow all contracts and reporting requirements with the Sunshine State Health Plan and Dept. of Children and Families and ensure HEDIS measures are met.
· Ensure the Behavioral Health Specialist responds to and assists with the management of in-patient and hospital notifications to HFC and all other appropriate parties
· Ensure the Inter Agency Specialist DJJ completes the Comprehensive Behavioral Health Assessment (CBHA) within the timeframe required by the Medicaid handbook
· Ensure the Nurse Care Specialist schedules and manages staffings for children who have been assessed; recommended or placed in medical foster care, deemed medically complex or fragile or who have ongoing medical or dental needs which impact their care while in the dependency system
· Work in partnership with the Medicaid Managed Care Agency (MMA) and the Agency for Health Care Administration (AHCA) through the approval process for services
· Provide consultation to case management on preparing appropriate information, including requests for initial suitability assessments; 60-day suitability assessments and 90-day suitability assessment reviews.
· Complete records review for in state and out of state admission consideration into residential mental health or substance abuse treatment
· Ensures that the clinical team schedules the appropriate multi-disciplinary staffings for children who have been assessed and recommended for STFC; STGH; SIPP/ RTC and substance abuse programs.
· Participate and if needed facilitate Baker Acts staffings for youths receiving case management services.
· Monitors the recording and quality of all behavioral health multi-disciplinary staffing notes
· Monitor and track behavioral health referrals for higher levels of care, such as Specialized Therapeutic Foster Care (STFC), Specialized Therapeutic Group Care (STGC) and SIPP, including the suitability assessment process
· Monitors the processing and tracking of medical case management referrals for assistance or referral to the MMA for medical case management
· Provides oversight by participating in or facilitating behavioral health multi-disciplinary staffings for those children who have been assessed and recommended for mental health or substance abuse programs
· Provides oversight by participating in or facilitating discharge planning staffings and participate in treatment team meetings and treatment plan staffings for children who are placed in therapeutic and/or behavioral health settings to ensure ongoing authorization and/or need for ongoing funding by HFC
· Participate and attend necessary Local Review Team staffings, State Review Team, APD or other MDT related staffings, court hearings and other meetings
· Perform special projects as assigned
