About the Role :
We are seeking a Clinical Operations Manager to oversee the daily operations of our home health site. This role is vital in ensuring the quality of patient care and supporting the relationship between referral sources, patients, caregivers, and employees. Your responsibilities will include :
- Overseeing the individualized care plans for clients, ensuring they are executed as written and reassessed as necessary.
- Ensuring complete documentation for all patients transferred to inpatient facilities.
- Managing the site when the Executive Director or Administrator is unavailable.
- Overseeing clinical, administrative, and recordkeeping functions efficiently.
- Participating in on-call schedules and providing skilled bedside nursing care in patients' homes.
