Overview

We are looking for a Team Coordinator Home Health to join our team. This position will directly report to the Administrator or Executive Director and is responsible for administering day-to-day operations of assigned home health programs/site and coordinates services of all team members.

• Schedules visits and communicates with field staff, patients, physicians, etc. to maintain proper care coordination and continuity of care.
• Manages all components of the calendar-changing of service codes, visit dates, clinicians.
• Ensuring staff are completing visits in a timely manner.
• Handles rescheduled, declined, missed, and reassigned visit requests

About You

• High School Diploma or GED
• Knowledge of roles of all disciplines providing home health services
• Excellent patient assessment skills
• Thorough knowledge of managed care principles, regulatory guidelines (i.e., Medicare, Medicaid, ACHC, and human resource), management principles.
• Strong verbal and written communication skills.
• Detail- and deadline-oriented with the ability to prioritize and multi-task
