We are seeking a skilled Care Specialist to join our team at Heartland Hospice.
Job Description
The Care Specialist will provide psychosocial support to patients and their families, utilizing professional training and personal judgment in monitoring the psychosocial assessment process. This role is ideal for individuals who possess a strong understanding of hospice care and the special needs of terminally ill patients and their families.
Responsibilities
- Conduct comprehensive psychosocial assessments of patients and their families to identify needs, goals, interventions, and services
- Develop and implement plans of care that address the unique needs of each patient and family
- Provide ongoing support and counseling to patients and their families, addressing issues related to illness, loss, and death
Requirements
- Masters Degree in Social Work from a CSWE accredited School
- Licensure as required by the state in which the hospice is located
- At least one year of experience as a Social Worker in a healthcare and/or hospice setting, with three to five years preferred
