Overview :
We are seeking a Hospice Patient Care Leader to join our compassionate team. In this pivotal role, reporting directly to the Executive Director, Administrator, or Senior Patient Care Manager, you will oversee and enhance our hospice program, ensuring high-quality patient care and a supportive environment for both staff and families.
- Manage and prioritize rescheduled, declined, missed, and reassigned visit requests, exercising clinical judgment and staffing decisions.
- Provide timely, professional, and compassionate responses to phone referrals and inquiries, including triaging medical concerns within your scope of practice.
- Prepare for, facilitate, and participate in the Interdisciplinary Group (IDG) meetings, presenting patients for review, coordinating minutes, and collaborating with care team members to monitor changes in patient conditions and care plans.
