Overview : Make a Meaningful Difference
At Heartland Hospice, we believe in providing personal care, palliative care, and hospice services that offer dignity, grace, and love. We are looking for a full-time Hospice RN Case Manager to work Monday through Friday during daytime shifts.
In this role, you will :
- Report directly to the Administrator, Executive Director, or Patient Care Manager Senior while maintaining primary management of each patient's care.
- Serve as the primary liaison among physicians, patients / families, and the hospice team.
- Deliver compassionate direct patient care services to hospice patients and support their families while managing a reasonable caseload.
- Empower caregivers by providing education and resources for caring for their loved ones.
- Collaborate with a dedicated interdisciplinary team of LPNs, CNAs, and support staff to ensure comfort and dignity for patients.
- Assess patient and family needs, assisting in the development and execution of personalized care plans.
- Maintain accurate and thorough clinical records for all assigned patients.
- Engage in quality assessment and performance improvement initiatives.
