We are currently looking for a Home Care Coordinator to join our Heartland PACE Team. Heartland PACE delivers a Program of All-Inclusive Care for the Elderly (PACE) that helps older adults live a meaningful, independent life at home with grace and dignity.
Position Summary: Under the direction of the Director of Clinical Services or Home Care Supervisor, performs PACE assessments of participants in their home environment and coordinates plan of care with appropriate resources. Participates in determining the need for and developing home care plans with the interdisciplinary team. Acts as the liaison between the contracted home health agency and the PACE program. Maintains the participant’s medical record with thorough documentation. Communicates participant’s changes and progress to appropriate staff.
Basic Qualifications:
Desire to serve frail older adults primarily in a community-based setting as a member of an interdisciplinary team.
Education/Training/Certifications:
· High School diploma or equivalent required
· Licensed nurse (RN or LPN) preferred, or certified Home Health Aide required.
· BLS required
· Current driver’s license and proof of auto insurance required
Experience:
· At least one year experience working with the elderly required
· Experience working with the elderly in home environment strongly preferred

· Skilled in identifying problems and recommending solutions.

· Understanding of the principles of diversity and inclusion and support their incorporation into practice.
· Familiarity with management information systems and electronic health records
General Responsibilities:
· Perform assessments of participants in their home environment to determine their individual needs.
· Work collaboratively with the interdisciplinary team (IDT) to develop and implement the comprehensive plans of care for participants.
o Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
o Participate in all interdisciplinary team meetings
o Assist the interdisciplinary team members in understanding the self-care and functional needs related to the participant’s home situation.
· Perform the duties of Home Care Coordinator on the IDT. Perform the duties of other IDT members based on professional licensing, competencies, and experience as needed.
· Actively participate in quality improvement projects / meetings.
· Function as a member of the interdisciplinary team. Maintain regular attendance at, and participate in interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24 hour care delivery.
· Assure that in-home staff provide supportive, personal and restorative care to participants based on the plan of care.
· Responsible for the coordination of personal care activities of participants in the home.
· Serve as a liaison between PACE and contracted Home Health services.
· Manage and prioritize homecare staffing needs to reflect actual enrollee homecare needs. Provide this information to the interdisciplinary team.
· Follow all Policies and Procedures and OSHA safety guidelines.
· Maintain the confidentiality of all company procedures and information about participants, clients or families.
· Handle potentially infectious specimens with appropriate biohazard precautions, and practice standard precautions.
· Participate in continuing education classes and any required staff and training meetings. Maintain professional affiliations and any required certifications.
· Perform other duties as required or requested.
