Heartland PCA is seeking a qualified professional (QP) — an RN, LPN, or LSW — to take charge of overseeing and launching our Veterans Services program. This role involves leading a team of caregivers to help veterans live independently in their homes. You will provide training, supervision, and ongoing evaluation of care -- ensuring clients' needs are met in alignment with program goals. As the Veterans Services Program Lead, you will have a key role in shaping this initiative, offering a role that includes developing care plans, ensuring quality service, and supporting both clients and caregivers in the field.
Duties:
- Oversee the launch of Heartland's Veterans Services program, ensuring smooth implementation and ongoing success
- Supervise, train, orient, and evaluate caregivers, providing leadership to ensure high-quality care for veterans.
- Develop and maintain care plans tailored to a client's individual needs.
- Travel to clients' homes and office locations as necessary for supervision, training, and care plan development.
- Maintain open communication between clients, caregivers, staff, and case managers to ensure comprehensive and coordinated care.
- Evaluate the effectiveness of services and adjust care plans as necessary to improve outcomes for veterans.
- Provide guidance and support to caregivers to alleviate caregiver burden and help Veterans maintain independence at home.
Requirements & Qualifications:
- Must be an RN, LPN, or LSW, or hold an equivalent qualification as described in applicable state and federal regulations.
- Must be at least 18 years of age.
- Must be willing to oversee a team and travel throughout the region to visit clients' homes and office locations.
- Must have demonstrated ability to work with minimal supervision, make sound decisions, and manage team performance effectively.
- Must have strong interpersonal communication skills and the ability to work well with clients, caregivers, family members, and other healthcare professionals.
- Must be dependable, tactful, and able to follow and enforce guidelines and policies effectively.
- Valid driver's license and proof of vehicle liability insurance.
- Must have a clear background with no history of jeopardizing the health and welfare of vulnerable adults through abuse or neglect (must pass a state and FBI background check).
- Must possess and maintain good physical and mental health to perform the essential duties of the role.
