Care Coordinators provide on-site and community-based care coordination for participants of the structured Cognitive Behavioral Curriculums provided to a criminal justice population at KDOC Correctional Facilities. These services include facilitating the integration of core components of any cognitive behavioral intervention with focus on substance misuse. Heavy emphasis is placed on skill building activities to assist with cognitive, social, emotional, and coping skill development. Requires passing pre-employment background checks and drug screen.
Duties & Responsibilities:
Implementation of a set curriculum:
- Attend specialized trainings by the University of Cincinnati, to understand the Cognitive Behavioral Curriculum Interventions for the Substance Misuse program, and/or the AIR program, both of which are developed for use within the criminal justice environment.
- Complete required forms and paperwork as instructed in a timely manner.
- Assist Cognitive Behavioral Instructors with providing the core components of the assigned curriculum, as defined by the University of Cincinnati, to identified participants in one on one and group sessions.
- Assign homework for skill development to participants, when assisting with curriculum instruction.
- Plan and execute activities on a group and individual basis as defined by the assigned Curriculum.
- Maintain a personal presence at the correctional facility that reinforces health, safety, and participation in accordance to the project design.
- Coordinate with reentry/release planning and ensures clients have case management during the Cognitive Behavioral classes/program, and have a seamless transition of services upon release.
- Coordinate continued care with parole upon client release from the facility.
- Track and document client and program outcomes and provide reports to Clinical Supervisor, Executive Team, and funders as requested.
Program Coordination
- Coordinate client care with the unit team counselor and any other staff in the facility who are working on the case
- Assist the Re-Entry Supervisor and Cognitive Behavioral Instructors to establish a referral and screening process to determine program eligibility, using screening interviews to ensure eligibility of participants.
- Screening of individual candidates requires analysis and discernment of whether the candidates are appropriate for initial participation in the program based on cognitive level, functioning and substance use history.
- Review the individual candidate’s criminal history and determine whether appropriate based on knowledge and understanding of the unique population and environmental circumstances.
- Monitor and report on achievement of output and outcome goals for the project by continued oversight and analysis of participant’s program involvement.
- Participate in interactive required trainings and meetings to determine the comprehensive evaluation of program components and outcomes.
Community Care Coordination
- Connect clients to needed services within the community and continued care coordination to address system barriers that are often present for this population;
- Assist client with obtaining safe sober housing; accessing mental health services including medication management;
- Connect clients to substance abuse services such as ROSC, formalized treatment programs, community support groups, and/or recovery coaches;
- Address physical health issues that result from chronic substance use
- Assist clients with the application for state and federal benefits, as needed.

EDUCATION AND EXPERIENCE:
- Bachelor’s degree and/or work experience in the substance use/mental health field, cognitive behavioral education, social services or criminal justice profession preferred.
- Experience working with diverse communities including those with substance misuse and criminal populations preferred.
- Experience and ability to use multiple computer applications, including but not limited to:
- Microsoft Office Suite applications including Word, Excel, Outlook and Power Point
- Ability to document client progress in electronic medical record
- Internet based database applications
