Intensive Care Coordinator (ICC) - KC Metro
Intensive Care Coordinator (ICC) provide case management and care coordination services to individuals with substance use disorders and co-occurring mental health diagnoses. The key to the success of this program is connecting ICC clients with needed services within the community and continued care coordination to address system barriers that are present.
ICC’s serve as a central point of contact and work to facilitate effective communication with the multiple stakeholders often involved with this client population (hospital social workers, Community Mental Health Centers, DCF workers, Probation/Parole & Law Enforcement Officers, Primary Healthcare Providers, etc.) to prevent or reduce the need for admission to hospitals, detoxes or jails by assisting the client to get into needed services.
Primary Goals of ICC
Avoid unnecessary and inappropriate admissions to State Hospitals, by connecting ICC clients to resources in their home communities;
Reduce the amount of bed days used when admission to a hospital is needed;
Improve quality of life by assisting ICC clients obtain housing and services.
Qualifications:
Licensure through the Behavioral Sciences Regulatory Board of KS is preferred.
Consideration given to those who have extensive experience in behavioral health working with co-occurring mental health or homeless populations and/or those working towards degree and licensure.
- LAC – Licensed Addictions Counselor
- LMAC – Licensed Masters Addictions Counselor
- LCAC – Licensed Clinical Addictions Counselor
Energetic, self-starter who likes to work autonomously in the community, meeting clients in a variety of locations
Excellent interpersonal skills with the ability to communicate effectively through verbal, electronic and written communications
Ability to evaluate statistical data and attention to detail is required
Adaptable to change and willingness to adopt new organizational and technology skills
Experience and proficiency using multiple computer applications is required, including but not limited to:
- Microsoft Office Suite applications including Word, Excel and Outlook
- Ability to document client progress in electronic medical record
- Internet based database applications
A valid driver’s license, dependable transportation, and proof of auto insurance must be maintained
Bi-lingual skills a plus
