What are the responsibilities and job description for the House Manager position at Heartland Supported Living Inc.?
Heartland Supported Living, Inc. is looking for individuals who are energetic with a positive attitude to fill our House Manager positions. CPR/First Aid, and L1MA is preferred. Must have OPEN availability Monday through Friday (will be scheduled 3pm-11pm shifts regularly) and able to work 7am-7pm Saturday and Sunday (every other weekend).
Job Duties Include:
- Order medications, refills, and PRN
- Pick up from the pharmacy, write in on the MAR and notify all staff
- Checks the MAR, case notes and log sheets during each shift to assure completed
- Prepare and turn in all medications for destruction by the nurse
- Assure all receipts are turned in, correctly documented and log is signed by staff and consumer
- Cover open shifts when necessary
- Participate in monthly reviews
- Provides on the job training for new staff shadowing in the home
- Meets with support staff who need coaching/retraining
- Meal planning for specialized diets
- Communicate with behavior therapist between appointments when issues arise
- Plan and organize daily activities in accordance with the consumer’s support plan
- Maintain the household and grocery shopping for the home
- Reports to the DDP all emergencies immediately and reportable information during business hours.
- Assures consumer mileage is tracked and does not exceed the monthly limit
- Assures all adaptive equipment is cleaned and maintained
- Accompanies consumers to all doctor appointments and assures paperwork is completed and submitted to the office.
- Serve one week in on call rotation with other House Managers and DDP
- Must respond to calls within one hour
- Assisting with consumer behaviors when needed
- Participates in annual ISP meeting
- Documents thorough case notes that address the outcomes as covered in the ISP and their progress. Documentation also expresses any and all concerns
- Attends medical appointments and is knowledgeable of the consumer’s medical history, current medical condition(s) and changes in medical condition.
- Reports on any health/mental health improvements, declines or other concerns that need to be addressed with the practitioner
- Makes sure the home has plenty of food/drinks, medical supplies and medications.
- Schedules monthly fire drills and quarterly severe weather and earthquake drill and assures they are completed
- Follows the training checklist/guide when training all new staff who shadow with their consumer
- Maintains consumer property inventory
- Provides direct consumer care including transportation and has a working vehicle at every shift.
- Delegates duties to other staff as appropriate.
